There are two steps to book an appointment.  

First we need you to fill out a new client intake form so we can learn about you. STEP 1

Once your form is complete, you will be automatically redirected to the Holistic Health Link website where you must fill out the "GEHA LifeCare Agreement FORM". STEP 2

We will reach out to you via email, text and/or phone to confirm your appointment request.
